SUPERBILLS FOR SELF-BILLING PURPOSE

Santa Fe Osteopathic Medicine does not provide insurance billing services, however, some insurances allow patients to self-bill for services to get reimbursed for all or a portion of their visit. Upon request, our office can provide a detailed super-bill with appropriate procedure and diagnostic codes should you want to submit your visit to your insurance directly to seek reimbursement. If you have any questions about your insurance’s self-billing policies and procedures, please contact your insurance directly.

MEDICARE

Santa Fe Osteopathic Medicine has opted out of Medicare. If you are a Medicare-eligible patient, you cannot submit your visit to Medicare or any supplemental plan. This includes Medicare plans managed through another private insurance. If you are Medicare-eligible, you will be asked to sign a Medicare Private Contract prior to your visit. Please visit the Medicare website to learn more about receiving care from a provider who has opted-out of Medicare.

Peptide Joint Injections (Ultrasound Guided) — $550 per treatment

Perineural Injection Treatment (PIT) — $250 per treatment

Platelet Rich Fibrin (PRF) Joint Injections (Ultrasound Guided) — $750 per treatment

Platelet Rich Fibrin (PRF) Facial (Microneedling)— $550
